By consenting to the performance of COVID-19 testing by Napa Biomedical Services, Inc., dba Neurovations, dba N3 Laboratories (“N3 Labs”), you agree to the following authorization. Please read the item below carefully.
I hereby authorize N3 Labs and/or Neurovations, its parent company, to contact me in the future to
seek my participation in clinical research projects, such as Covid vaccine studies. I understand and
acknowledge that N3 Labs will never sell or unlawfully transfer my health information without my prior explicit authorization.
I further understand and acknowledge that this authorization DOES NOT commit me to any research study and that I may decline participation any time I am contacted by N3 Labs or its parent company, Neurovations, about a clinical research project.
