By consenting to the performance of COVID-19 testing by Napa Biomedical Services, Inc., dba Neurovations, dba N3 Laboratories (“N3 Labs”), you agree to the following acknowledgment, waiver, and authorization. Please read the item below carefully:

I hereby authorize N3 Labs to provide notice of my COVID-19 test results and other relevant health information to me through electronic means, including e-mail or SMS text communications.

I understand and acknowledge that such forms of electronic communication are generally unencrypted, which means that communications containing my health information can be read and intercepted by third parties during transmission.

I further understand and acknowledge that N3 Labs will implement additional safeguards when communicating with me electronically, including, but not limited to, checking therecipient’s contact information for accuracy and limiting the amount or type of information disclosed.
